Music Therapist

The Music Therapist is responsible for providing therapeutic interventions to address physical, emotional, cognitive, and social needs of individuals. The Music Therapist helps to ensure patient support is coordinated and managed appropriately.

Essential Job Functions/Responsibilities

  • Evaluate patient’s functional status and assess patient’s response to music, music preferences, music skills and cultural factors.
  • Develop, administer, and coordinate music activities and programs in accordance with state and federal regulations.
  • Implement music activities that facilitate non-musical goals (e.g. sensory stimulation, memory recall, positive failure, socialization, etc.)
  • Monitor the condition of musical instruments.
  • Work as part of the interdisciplinary team to care plan in determining therapeutic objectives and goals.
  • Develops, oversees, and coordinates staff wellness/support activities with leadership team.
  • Promotes hospice referrals in the health care community.
  • Other duties as assigned by Supervisors.
  • Complete documentation in accordance with CMS and HOL guidelines and regulations.
  • Adhere to all HOL policies, procedures, and standards of conduct as noted in the employee handbook.
  • Documentation, visits and mileage will all be entered daily when visit is completed.

 

Position Qualifications

  • Music Therapist Board Certified (MT-BC)                                                                  
  • Two years of music therapy experience and/or hospice experience.
  • Must be a licensed driver with an automobile that is insured in accordance with state guidelines and is in good working order.
  • Must have liability insurance.
  • Has excellent observation, verbal, and written communication skills.

Chaplain / Bereavement Support Coordinator

The Chaplain/ BSC is responsible for providing therapeutic interventions to address physical, emotional, cognitive, spiritual, and social emotional needs of individuals. The Chaplain/ BSC helps to ensure patient spiritual support and emotional support is coordinated and managed appropriately during care as well as for bereavement care.

Essential Job Functions/Responsibilities Chaplain

  • Assures spiritual assessment of patients and families in the hospice program are rendered in a timely manner.
  • Facilitates the development in the interdisciplinary team/group plan of care to meet the identified spiritual needs.
  • Provides direct spiritual care to patients as well as families/caregivers.
  • Serves as a liaison and support to community chaplains and spiritual counselors.
  • Maintains records of spiritual care services for quality assurance, and program development.
  • Performs documentation of visits and community visits ongoing and timely.
  • Attends patient care conferences and IDT meetings as a member of the interdisciplinary team.
  • Recruits’ community chaplains, spiritual counselors, spiritual care volunteers that can adequately help to support the patient, family and caregiver needs by developing community contacts and offering education through congregations about hospice care.
  • Provides funeral or memorial services for patients as requested by families.
  • Plans memorial services and assists in supervision of spiritual volunteers.

 

Essential Job Functions/Responsibilities Bereavement Coordinator

  • Coordinates the assessment and delivery of grief counseling needs and services (one to one, groups, and ongoing follow-up) to ensure that timely and appropriate services are provided to families/patients and caregivers.
  • Plans for Bereavement program development, expansion, and refinement through annual evaluations of services.
  • Provides bereavement support to hospice personnel coping with work related grief.
  • Provides bereavement information and referral services to callers from the community.
  • Assist as needed in evaluating and planning services to meet hospice personnel support needs.
  • Functions independently but seeks and accepts guidance from other members of the interdisciplinary groups or from members of the community.
  • Participates in hospice activities such as in-service education, hospice personnel meetings and relevant committees.
  • Performs documentation and duties as deemed by hospice CMS and COP guidelines.
  • Performs other duties as deemed necessary.

 

Position Qualifications

  • Graduate of an accredited seminary school of theology (BD, MDiv or equivalent theological degree or master’s in social work with qualifications of experienced clinical grief counseling.
  • Experience with death and dying.
  • Must be a licensed driver with an automobile that is insured in accordance with state guidelines and is in good working order.
  • Comfort with an interfaith setting that is willing to accept different lifestyles, cultures, beliefs, and values.
  • Ability to network with community clergy and congregations.
  • Knowledge of and commitment to hospice philosophy of care.
  • Has excellent observation, verbal, and written communication skills.
  • Understanding of grief responses and experience with counseling individuals in groups regarding loss.
  • Hospice experience preferred.

Registered Nurse

The successful candidate should possess:

  • Valid Michigan RN license
  • Valid Michigan driver’s license
  • Critical thinking skills
  • Compassion and kindness
  • Previous hospice experience
  • Previous experience working in or with long term care facilities
  • Strong grasp of medication and alternative therapy management
  • Ability to handle crisis situations, including home hospice admissions, pain/symptom management, and informational visits to families and patients who are seeking hospice care
  • Nurse triage skills, providing person centered care
  • Experience with documenting using electronic medical records software
  • Excellent communication skills
  • Enjoys collaborating with other team members and outside teams at facilities to provide exceptional patient care
  • Customer-service oriented

Salary: $30-35/hour

Home Hospice RN Case Manager

A RN Case Manager plans, organizes and directs hospice for their patients assigned on their care caseload.  This nurse is experienced in nursing, with emphasis on community health education/experience. A RN Case Manger builds from the resources in the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Essential Job Functions/Responsibilities

Patient Care

  • Completes an initial and comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Assesses and evaluates patient’s status by:
    • Writing and initiating plan of care
    • Regularly re-evaluating patient and family/caregiver needs
    • Revising the plan of care as necessary
    • Initiates the plan of care and makes necessary revisions as patient status and needs change.
    • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.

Communication

  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Communicates with community health related persons to coordinate the care plan.
  • Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the other hospice Interdisciplinary Group Members.
  • Provides and maintains a safe environment for the patient.
  • Assists the patient and family/caregiver and other team members in providing continuity of care.
  • Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver.
  • Attends Interdisciplinary Group meetings.

Additional Duties

  • Participates in on-call duties as defined by the on-call policy.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Actively participates in quality assessment performance improvement teams and activities.

Job Types:
